In-home behavioral health
Through the AIDS Foundation of Chicago, we are able to provide integrated behavioral services that fill gaps in care for our clients. The behavioral health programming provides in-home mental health and substance use counseling delivered by licensed clinicians for clients with problematic substance use and/or mental health disorders. Using a trauma-informed model as their foundation, Behavioral Health Specialists utilize a variety of evidenced-based practices including Harm Reduction, Motivational Interviewing and Behavioral Therapies. The Behavioral Health Specialists also attend Systems Integration Team meetings, which bring together the clinicians and case managers, to facilitate case consultation; provides individual client assessments and feedback to case managers; communicates with clients’ medical providers about behavioral health needs; and assists with crisis intervention in the field.
SOAR (SSI/SSDI Outreach, Access and Recovery)
We have a trained SOAR specialist who specifically assists people experiencing or at risk of homelessness who also have a mental illness, medical impairment, and/or a co-occurring substance use disorder. The program’s goal is to help clients successfully gain access to SSI and SSDI benefits so they can focus on achieving other health and wellness goals.
