Coordinated Entry

The Center for Housing and Health is a subrecipient for the Chicago Coordinated Entry System (CES). CES is the largest, centralized referral network to affordable housing for people experiencing homelessness in our community. In being a subrecipient to the Lead CES Agency, All Chicago, we administer the work of Skilled Assessors and Housing Systems Navigation. Skilled Assessors complete the Coordinated Entry Housing Assessment with participants experiencing homelessness in our community. The Housing Assessment is the entry point to CES. On the other end, the work of Housing Systems Navigation ensures households who have been matched to housing through CES are successfully housed by providing wraparound services and serving as a liaison between the participant and their housing program.
